- After your payment is processed, you will be directed to the intake form.
- Complete the form with your personal and medical information.
- If you have insurance, double-click the yellow "Book Online" button to upload your insurance information and ID.
- Complete the intake form with all necessary details.
Step 4: Important Information
- Deposit Management: We hold your deposit until we receive payment from your insurance company. Alternatively, we can provide you with a superbill, which you can use to submit a claim to your insurance company yourself.
